Chemical and Physical Properties
- Chemical Name: Semaglutide
- Formula: C₁₈₇H₂₉₁N₄₅O₅₉
- Molecular Weight: Approximately 4113.58 g/mol
- Structure: A modified analogue of the human GLP-1 peptide with 94% sequence homology, featuring specific amino acid substitutions and a fatty acid chain attachment to enhance albumin binding and prolong its half-life.
- Physical Form: Available as a clear, colorless or almost colorless, isotonic solution for injection or as white to light yellow oval-shaped tablets.
Mechanism of Action
Semaglutide works by mimicking the naturally occurring GLP-1 hormone. It selectively binds to and activates GLP-1 receptors in the pancreas, brain, heart, vasculature, immune system, and kidneys.
- Glucose Regulation: Stimulates glucose-dependent insulin secretion from pancreatic beta cells and suppresses glucagon release from alpha cells when blood glucose is high, thereby reducing both fasting and postprandial glucose levels.
- Appetite Regulation: Acts on GLP-1 receptors in the hypothalamus to reduce hunger, increase feelings of satiety (fullness), and decrease food cravings, leading to reduced energy intake and body weight loss.
- Gastric Emptying: Delays gastric emptying, which contributes to its blood glucose-lowering and weight loss effects.
- Cardiovascular & Renal Effects: Exerts beneficial effects on cardiovascular risk factors (e.g., blood pressure, lipids) and has anti-inflammatory properties. It has been shown to reduce the risk of major adverse cardiovascular events in certain patients.
Pharmacokinetics
- Bioavailability: High (approx. 89%) for subcutaneous injection; very low (0.4–1%) for oral tablets (which are co-formulated with an absorption enhancer, SNAC).
- Half-Life: Long elimination half-life of approximately 1 week (about 165–185 hours) due to strong albumin binding, allowing for once-weekly dosing for injections.
- Elimination: Primarily through metabolism via proteolytic cleavage and beta-oxidation, with metabolites largely excreted in the urine and feces.
Research Applications
Semaglutide is a key subject in various research areas beyond its approved clinical uses, including:
- Obesity and Weight Management: Extensive studies (e.g., the STEP program) investigate its efficacy in chronic weight management in adults and adolescents.
- Cardiovascular and Renal Disease: Research focuses on its benefits in reducing the risk of heart attack, stroke, and kidney function decline in individuals with type 2 diabetes and existing conditions.
- Neuroprotection: Investigated for potential neuroprotective properties and therapeutic effects in neurodegenerative diseases like Alzheimer’s and Parkinson’s due to its ability to cross the blood-brain barrier and reduce neuroinflammation.
- Metabolic-Associated Steatohepatitis (MASH): Studied and recently approved for the treatment of non-cirrhotic MASH with moderate to advanced liver scarring.
- Polycystic Ovary Syndrome (PCOS): Research shows potential benefits in managing PCOS symptoms, including weight loss and improvements in insulin resistance and hormonal balance.
